PURCHASE OF NITROUS OXIDE CARTRIDGES ACKNOWLEDGEMENT
(N2OAGR-v01.04.01)
In consideration of agreement by Elite Gas, LLC to sell nitrous oxide cartridges to Customer, Customer warrants and represents that such cartridges are intended for use and/or resale/distribution exclusively for the purpose of food preparation or industrial use. Customer has been informed and understands the following:
• Inhalation of nitrous oxide outside of a clinical setting may have dangerous health effects.
• It is a violation of state law to possess nitrous oxide or any substance containing nitrous oxide, with the intent to breathe, inhale, or ingest it for the purpose of intoxication.
• It is a violation of state law to knowingly distribute or dispense nitrous oxide or any substance containing nitrous oxide, to a person who intends to breathe, inhale, or ingest it for the purpose of intoxication.
Furthermore, I, the Customer, certify:
• I am at least 21 years of age
• I will provide a valid government-issued photo identification
Customer further warrants and represents that, if such nitrous oxide cartridges are purchased for resale and/ or distribution, Customer will ensure that purchasers from Customer are 21 years of age or older and intend the product exclusively for the use(s) designated above; and shall obtain from such purchasers and maintain a document signed by such purchasers setting forth the foregoing information, and complying with all the terms of California Penal Code section 318e. By accepting these terms, Customer further acknowledges reading and understanding the following, and agrees and warrants that if such nitrous oxide cartridges are purchased for resale or distribution, Customer will obtain a document signed by the purchaser setting forth the following information and complying with all the terms of California Penal Code Section 381c and 381e.
